An 82-year-old man with severe mixed Alzheimer's and vascular dementia died of pneumonia while detained under a Mental Health Act Level 2 Inpatient Treatment Order at a specialized psychogeriatric facility. He had been transferred there after failing management in mainstream nursing homes due to significant behavioural and psychological symptoms. Palliative care was initiated after discussion with family regarding end-of-life wishes. Following treatment for suspected respiratory tract infection at hospital, he was discharged back to the facility where he deteriorated and died. Investigation found no evidence of abuse or neglect, and his custodial circumstances did not contribute to death. Care provided was deemed appropriate.
