Karl Melo-Richards, a 25-year-old man with a long history of depression and suicidal ideation since age 14-15, went missing on 2 September 2016. Despite extensive police investigations and searches, his body was never found. The coroner concluded on the balance of probabilities that Karl died by intentionally jumping from cliffs near Magic Point in Malabar, likely between 1:45pm and 4:30pm on 2 September 2016. Karl had made previous suicide attempts, multiple visits to psychiatric facilities, and was receiving ongoing mental health treatment. Clinical lessons include the importance of consistent mental health care, recognising warning signs (sleep disturbance, social withdrawal, farewell gestures), maintaining therapeutic continuity when patients discharge, and considering residential monitoring for high-risk individuals.
