Finding into death of Richard James Vanderhelm

Richard Vanderhelm, a 42-year-old man with a long history of substance dependence, disappeared on 21–24 May 2009 while undergoing community-based detoxification from methadone, cannabis and benzodiazepines at Somers Caravan Park. His body has never been recovered, but evidence suggests likely accidental death, possibly drowning near Somers Beach. An AHPRA investigation found his GP, Dr I., had engaged in unprofessional conduct by failing to adequately diagnose the extent of drug dependence and subsequently prescribing excessive doses of drugs of dependence without proper safeguards. The case highlights risks of managing complex substance use disorder in general practice without adequate specialist oversight, particularly regarding dose escalation and withdrawal planning.
