Cody Watson, 19, died by hanging following childhood sexual abuse, family violence, and inadequate trauma-informed care. She disclosed abuse at 15, subsequently developing borderline personality disorder, anxiety, depression, and substance use disorder. Although GPs and mental health services (Austin Health YETTI, Headspace) provided appropriate risk containment and safety planning, they did not offer trauma-focused cognitive-behavioural therapy or other evidence-based trauma-specific treatments. The coroner found that best-practice care for child sexual abuse survivors requires specialized trauma-informed assessments and therapies, which are severely limited in public mental health services. Key clinical lessons include the need for early specialist involvement, trauma-informed practice across all disciplines, and better coordination between primary care, mental health, and sexual assault services.
