Coroner's Finding: SLOAN Adam Edward John

A 15-year-old male died from right haemothorax due to lung lacerations sustained in a high-speed motor vehicle collision. The vehicle, driven by an unlicensed 15-year-old driver with faulty brakes, crashed into a tree after a party at which minors consumed alcohol provided by an adult. Emergency response was appropriate: rescue teams extracted him from severe entrapment within 100 minutes; pre-hospital care including intubation was excellent; hospital resuscitation in the Emergency Department was well-managed with appropriate use of chest drains; cardiothoracic theatre activation was timely. The injury itself (massive intrapulmonary bleed) was exceptionally rare and devastating; surgical intervention was attempted but massive re-bleeding in theatre proved unsurvivable. No clinical management failures were identified. The death was fundamentally attributable to the severity of the injury and the adult's provision of alcohol to minors and failure to maintain vehicle safety.
