Gary Mayrhofer, a 54-year-old man with cerebral palsy, intellectual disability, epilepsy and visual impairment, died from aspiration pneumonia following a stroke in September 2023 that caused right-sided paralysis and swallowing difficulties. He spent eight weeks in hospital before discharge to supported disability accommodation where he received care from Scope staff, Barwon Health, and community nursing. The coroner found no evidence of deficient care by Scope or treating clinicians. The death resulted from natural causes—a complication of his prior cerebrovascular accident. This case highlights the importance of appropriate swallowing assessment and management in post-stroke patients with dysphagia in supported care settings, and the need for coordinated interdisciplinary care between hospital, community health services, and disability support providers.
