Finding into death of Jack Elliot Atkinson

Jack Atkinson, 26 years old, died from mixed drug toxicity involving GHB/1,4-butanediol, methylamphetamine, cocaine, novel benzodiazepines (bromazolam, flubromazepam), nordiazepam, and ketamine. He had a history of substance use disorder since age 16, successfully completed rehabilitation in May 2022, but relapsed after an overseas holiday. His GP appropriately identified withdrawal symptoms and referred him to hospital in November 2023, prescribing Valium. The clinical lesson centres on the unpredictable and dangerous effects of novel psychoactive substances in unregulated drug markets, and the difficulty in preventing deaths when patients with established addiction resume illicit drug use despite treatment. Better access to addiction support services and harm reduction strategies may have been beneficial.
