Wendy Joy Morgan, aged 56, died from complications of acute on chronic haemorrhage into a suprachiasmal pilocytic astrocytoma. She had suffered an acquired brain injury in 2021 from hydrocephalus secondary to the brain tumour and was residing in specialist disability accommodation at the time of her death. She collapsed while being transferred to bed and could not be revived despite CPR and ambulance attendance. This was a natural death from a known neurological condition. The case highlights the importance of appropriate residential care planning for individuals with acquired brain injuries and seizure disorders, though no preventable errors were identified in this investigation.
