Wayne Joannou, 26, died from a gunshot wound to the head on 18 February 2005 when the Special Operations Group attempted his arrest. He was a known violent offender with a 10-year criminal history and substance abuse issues (heroin, amphetamines, benzodiazepines). Following his involvement in Brian Bottomley's murder on 2 February 2005, Joannou evaded arrest for 16 days while armed with a shotgun. Medical management was problematic: despite undergoing Naltrexone implant detoxification on 20 January 2005 with psychiatric support mentioned as available, there was no documented psychiatric assessment of acute paranoia or psychosis post-procedure. Joannou continued using cocaine and other drugs despite the implant. Critical clinical lessons include: the need for mandatory psychiatric follow-up post-detoxification (particularly given baseline paranoia and anxiety), documented monitoring for breakthrough drug use and mental state deterioration, structured aftercare engagement rather than reliance on patient compliance, and early escalation to mental health services when paranoid ideation emerges post-procedure.
