A 40-year-old woman with Moebius syndrome, cerebral palsy and intellectual disability died from complications of influenza A infection. She resided in specialist disability accommodation with 24/7 support. On 3 July, she displayed signs of illness with coughing and chest discomfort; her aunt advised against calling an ambulance due to previous help-seeking behaviour. The next morning she vomited and was hospitalised. Despite antibiotics and sepsis pathway management, her condition deteriorated with aspiration pneumonia and she died 4 days later. The coroner found no clinical management failings by disability support staff or hospital clinicians. The case highlights the clinical challenge of distinguishing genuine acute illness from behaviour-driven hospital access in patients with intellectual disability and communication difficulties, and the importance of documenting decision-making rationale when declining emergency transport.
