A 30-year-old man with untreated long-standing mental health difficulties died by intentionally driving his vehicle into an oncoming truck on 5 September 2017. He had experienced relationship stress, demonstrated poor emotional resilience, and made multiple calls expressing suicidal intent in the 90 minutes before his death. Despite police welfare checks and attempted location, he was not intercepted. The coroner found he had likely benefited from mental health professional assessment and treatment, which was never sought or documented. This case highlights the importance of recognising mental health distress in at-risk individuals, facilitating early professional mental health intervention, and ensuring adequate support for those with relationship difficulties and emotional dysregulation.
