A 92-year-old woman with multiple comorbidities (aortic stenosis, anaemia, atrial fibrillation, ischaemic heart disease) died of heart failure four days after admission to aged care. She had been hospitalised for three months following a fall, during which her cognitive function declined significantly. The coroner found she received appropriate care at all material times. The death was reportable because she was a protected person under guardianship with section 32 detention powers. The coroner made no recommendations. Clinical lessons include ensuring timely medical review in aged care settings and appropriate documentation of advance care planning such as 'do not resuscitate' orders.
