Charles Ross Maxwell Morgan, an 18-year-old Aboriginal man, died when struck by a train near Zillmere Railway Station in January 2012. He was moderately intoxicated with alcohol and cannabis, with recent cannabis use. The coroner could not definitively determine whether this was suicide or accidental death (falling asleep on tracks). Charles had a volatile on-and-off relationship with his girlfriend, had previously expressed suicidal ideation if she left him, and had risk factors including substance abuse and impulsive aggression. However, he appeared happy to friends that evening and had family support. Key clinical lesson: young people with substance abuse, relationship instability, and aggressive outbursts require comprehensive mental health assessment and crisis planning. The last witnesses provided incomplete and inconsistent evidence, limiting investigation. Better mental health follow-up and substance abuse intervention might have been beneficial.
