Female·retroperitoneal blood loss in a setting of anticoagulation therapy following hip replacement of the left hip

Valerie Kolts died from retroperitoneal blood loss following hip replacement surgery while on anticoagulation therapy. The coroner found no lack of care contributed to her death. Although some clinical signs were retrospectively identifiable as potentially related to the fatal condition, they were not readily obvious at the time and it is unclear whether earlier recognition would have changed the outcome. This case illustrates the challenge of recognising uncommon complications of anticoagulation therapy post-operatively and the difficulty of attributing preventability when outcomes are uncertain despite recognised symptoms.
