Inquest into the Death of Gary John Holcroft

Gary John Holcroft, a 30-year-old prisoner at Casuarina Prison Complex, died on 19 September 2001 from Hypertrophic Cardiomyopathy. He had a known heart murmur since childhood and Hepatitis C, and was noted to have elevated cholesterol and smoking concerns. In late August 2001, he requested medical review for cholesterol concerns and smoking cessation support. Blood tests were ordered and results reviewed in early September. He was found unresponsive in his cell at approximately 1810 hours and resuscitation was commenced immediately but proved unsuccessful. Post-mortem examination revealed markedly enlarged heart with aortic valvular stenosis. The coroner found the condition was unlikely to be discovered prior to death without advanced imaging (echocardiography), and that medical supervision and care were adequate, noting the deceased's own limited engagement with health concerns despite medical availability.
