Jake Garrett-Pratt, a 12-year-old boy, died from fatal head injuries sustained when he lost control of a poorly maintained ride-on lawn mower on a steep driveway. The rear axle drive chain broke due to inadequate maintenance, age, and wear, leaving the mower without effective braking. Contributing factors included: lack of mechanical inspection or maintenance before the child used it; absence of supervision; no safety helmet use; operation on unsuitable terrain; and use of equipment not designed for the purpose. The coroner found the parents' decision to permit unsupervised operation was unsound given the child's age, the vehicle's poor condition, and his known tendency to ride down steep slopes.
