Micheal Blutcher, aged 31, died from accidental fentanyl overdose in custody at Capricornia Correctional Centre Farm after injecting fentanyl extracted from a transdermal patch. The fentanyl was smuggled into the low-security facility in a soft drink can thrown over the external perimeter fence by unknown persons. He injected approximately 3ml of fentanyl extract and subsequently injected an additional share intended for another inmate, resulting in a lethal dose. The coroner found no evidence of intent to self-harm. Clinical lessons include recognition that fentanyl is 50-100 times more potent than morphine with a narrow therapeutic margin; the high-risk population for fentanyl misuse (males aged 30-39); and extraction from transdermal patches represents a common fatal administration method. Security and surveillance gaps at the perimeter fence allowed drug delivery to occur undetected over multiple weeks. The coroner noted this was a low-security rehabilitation facility where prisoners had reduced supervision during work duties, balancing rehabilitation goals against security risks.
