A 31-year-old woman with a long history of depression, anxiety, and PTSD died by hanging. She experienced multiple concurrent stressors: workplace sexual harassment and threats, endometriosis diagnosis requiring surgery, relationship breakdown, and housing insecurity. Clinical notes documented severe psychological distress (K10 score of 31 indicating severe mental disorder) yet she was cleared to return to work. She concealed suicidal ideation from family and healthcare providers, who assessed her as having no suicidal thoughts. Referred psychologists were never attended. The coroner found the death could not have been foreseen or prevented given her private nature and concealment of difficulties, despite receiving treatment and support offers.
