Joel Cauchi, suffering a psychotic relapse of chronic schizophrenia, carried out a rapid active armed offender (AAO) attack at Westfield Bondi Junction shopping center on 13 April 2024. Within three minutes, he fatally stabbed six people (Dawn Singleton, Jade Young, Yixuan Cheng, Ashlee Good, Faraz Tahir, and Pikria Darchia) and injured ten others. The inquest examined the preparedness and response of security, police, and emergency services to this unprecedented incident. Key findings identified deficiencies in security control room procedures, delayed activation of alerting systems, and insufficient initial radio communications. Inspector Amy Scott's rapid response and shooting of the offender prevented further casualties. The coroner recommended enhanced public awareness of 'Escape. Hide. Tell.' safety messaging, permanent implementation of two-operator CCTV control rooms, and improved emergency response procedures and training for crowded places.
