Finding into death of Wayne Daniel Holland

Wayne Daniel Holland, aged 61, died from urosepsis related to chronic suprapubic catheterisation. He had significant disability from congenital rubella and lived in specialist disability accommodation. He presented to hospital on 26 July 2024 with agitation, reduced urine output, and deterioration. A CT scan confirmed bladder infection and possible right lower lobe consolidation. His family had previously established a ceiling of care limiting treatment to ward-based care without ICU admission or resuscitation. When metaraminol infusion required ICU-level care, clinicians discussed this with his mother and sister who confirmed the pre-established ceiling of care. He was transitioned to palliative care and died peacefully. The coroner found no want of clinical management or care by either the SDA staff or Austin Hospital staff that caused or contributed to his death.
