Mr B, aged 75, killed his wife Mrs A by striking her to the head and asphyxiating her with a ligature, then set their house on fire resulting in his own death. Evidence indicates Mr B was experiencing undiagnosed mental health conditions, mood swings, and manic behaviour. The couple faced multiple stressors including health decline, relationship separation, financial disputes, and COVID-19 isolation. While healthcare providers were involved with the couple's medical needs (gastroenterology, general practice), no family violence concerns were identified and no prevention opportunities were apparent in their medical interactions. The coroner noted this case exemplifies homicide-suicide amongst elderly couples with documented premeditation through written letters.
