Mr Mehmet Hasan, 87, died from complications of subdural haematoma and fractured hip following an alleged traumatic incident involving staff at Lorikeet Lodge Aged Care on 12 June 2010. Following transfer to hospital, hip fracture surgery was performed, but he deteriorated and died on 23 July 2010. Key clinical lessons: (1) failures in proper escalation and investigation of patient safety concerns and elder abuse allegations can compromise patient care; (2) diagnosing injuries in dementia patients requires careful physical examination and appropriate imaging; (3) adequate supervision of junior staff and strong incident reporting culture are essential; (4) doctors have statutory obligations to report certain deaths to the coroner. The coroner identified systemic governance failures at the facility affecting incident management, staffing, and safety systems.
