Gregory Dean Coutts, a 46-year-old man with intellectual disability, spastic quadriplegia, and epilepsy living in a Department of Human Services group home, died from septic shock complicating pneumonia. On 3 October 2011, group home staff requested medical review when he presented with fever, increased coughing, and vomiting. His GP suspected severe aspiration pneumonia and arranged hospital transfer. At St Vincent's Hospital, he was found unresponsive and hypotensive with confirmed pneumonia. Despite aggressive fluid resuscitation and intravenous antibiotics, he failed to respond. After family consultation regarding poor prognosis, active treatment was withdrawn in favour of palliative care. The coroner found no evidence that care provision by group home staff, the GP, or hospital staff contributed to his death. The case highlights the vulnerability of patients with bulbar dysfunction to aspiration pneumonia and the importance of coordinated multidisciplinary assessment in managing complex disability.
