Finding into death of Miranda Louise Lynch

Miranda Lynch, a 46-year-old woman with severe cerebral palsy, spastic quadriplegia, epilepsy, and aspiration pneumonia history, died from sepsis of unknown origin. She was admitted to Box Hill Hospital on 20 April 2023 after two episodes of altered consciousness, presenting with bradycardia, hypotension, and hypothermia. Despite extensive investigations and treatment with intravenous fluids, hydrocortisone, and antibiotics, the source of sepsis could not be determined. She died 4 days later. The coroner found no preventable factors and accepted the forensic pathologist's opinion of natural death. The case highlights challenges in identifying sepsis source in complex disability patients with multiple comorbidities and the importance of prompt recognition and treatment initiation, though outcomes may be limited regardless of intervention timing.
