A 44-year-old woman with chronic back pain and depression died from mixed drug toxicity with aspiration of gastric contents. She had been prescribed multiple medications including amitriptyline, diazepam, codeine, and lorazepam, with post-mortem toxicology showing potentially lethal levels of amitriptyline and diazepam. Her GP had previously suspected medication misuse when she claimed lost prescriptions but maintained the therapeutic relationship to keep her engaged. The coroner found it impossible to determine whether death resulted from intentional overdose or unfortunate drug combination. The GP's management was deemed entirely reasonable; he could not have predicted this outcome and the patient appeared to be improving at their final consultation.
