Inquest into the death of Scott Clements

Scott Clements, a 36-year-old man with longstanding mental illness, depression, substance use disorder and multiple suicide attempts, was found dead at his home between 19 August and 14 October 2014. His body was in advanced decomposition, precluding determination of cause of death. He had recently relapsed into drug use including methamphetamine and prescription medications, and expressed suicidal ideation on 18 August 2014, with referral to crisis services. He attended Camperdown Health Centre on 19 August for assessment and denied current suicidal plans, though reported significant isolation and despair. He failed to attend a scheduled drug health appointment on 21 August and had no documented medical contact thereafter. The coroner could not determine whether death was by suicide, accidental overdose, or natural causes. Clinical lessons include the challenges of assessing suicide risk in patients with chronic suicidality, importance of follow-up after crisis assessment, and engagement of isolated patients with complex mental health and substance use comorbidities.
