A 28-year-old man with treatment-resistant paranoid schizophrenia and substance abuse died by hanging while detained under the Mental Health Act in a closed psychiatric ward. He had chronic auditory hallucinations commanding self-harm and had made previous suicide attempts. Despite being assigned 'close' observation level requiring 15-minute checks, the ward's actual checklist only provided hourly checks after 21:30 hours, deviating from policy. The coroner found that more intensive observations may not have prevented death given the inevitability of serious self-harm during stress periods. Key clinical lesson: maintain consistency between observation policies and actual practice, and recognise that management of highly symptomatic, treatment-resistant psychosis presents genuine clinical dilemmas without simple solutions.
