The case highlights critical gaps in clinical coordination: after February 2024, no single clinician took primary responsibility for her care. A GP trainee initiated a complex opioid rotation—a high-risk procedure requiring experienced oversight—without adequate follow-up or monitoring. Mrs Bridle experienced an 8-week gap without clinical review. SafeScript was not consistently checked by prescribers, and no Schedule 8 permit was obtained despite opioid duration and dosage requirements. Mrs Bridle self-managed her medications irregularly, likely losing opioid tolerance.
