Frederick Williamson, a 52-year-old man with chronic treatment-refractory schizophrenia, was found dead in the bathroom of his room at Austin Health's Secure Extended Care Unit with a plastic bag over his head, consistent with asphyxia. However, the coroner found on the balance of probabilities that Freddy's multiple facial and head injuries (involving blunt force trauma on different planes) were not self-inflicted and were consistent with assault by an unknown person.
