Finding into death of Scott Adam Brown

A 50-year-old man with Parkinson's disease died by hanging in a psychiatric inpatient unit while on a Temporary Treatment Order. His mental and behavioural symptoms were secondary to Parkinson's disease and its treatments rather than primary psychiatric illness. Key clinical lessons include: (1) complex patients with comorbid medical and psychiatric conditions require integrated multidisciplinary care plans rather than separate specialty plans; (2) information transfer between treating services was suboptimal; (3) environmental hazards on psychiatric wards, particularly high-risk items like skipping ropes usable as ligatures, must be actively managed; (4) exercise equipment brought onto wards requires formal monitoring processes; (5) managing the balance between adequate Parkinson's symptom control and medication-induced behavioural disturbances is extremely challenging and requires careful coordination. Alfred Health implemented recommendations addressing integrated care planning and high-risk item management.
