Jacob MacDonald, 51, died at the Forensic Hospital from combined effects of dilated cardiomyopathy with myocardial fibrosis and toxic levels of clozapine and aripiprazole. He was an involuntary forensic psychiatric patient with treatment-resistant schizoaffective disorder and obstructive sleep apnoea. While expert evidence confirmed excellent psychiatric and cardiac care, clinicians should note: (1) postmortem toxicology revealed prescription drugs at toxic-lethal levels unexplained by therapeutic doses, suggesting possible overdose; (2) sleep observations documentation failed to record respiratory status as required by policy; (3) cardiac monitoring protocols were actually exceeded standards; (4) the combination of antipsychotic cardiotoxicity, undiagnosed cardiomyopathy, obesity, and sleep apnoea created high arrhythmia risk, particularly concerning given drug-induced respiratory depression risk in OSA patients. The cause of elevated medication levels remained undetermined, highlighting the difficulty in monitoring medication compliance in forensic settings.
