HJ, a 76-year-old woman with chronic psychotic disorder and paranoid delusions, presented with abdominal pain, vomiting, and small bowel obstruction. Imaging revealed bilateral adnexal masses. Due to her lack of insight and refusal of interventions, she was placed under a Mental Health Act order. She underwent gastroscopy for diagnostic purposes, could not be extubated post-procedure, and was admitted to ICU with type 2 respiratory failure. Despite supportive care including non-invasive ventilation and vasopressor support, she deteriorated and was transitioned to palliative care, dying on 12 June 2021. The coroner found the care reasonable and appropriate, identifying no preventable factors. The cause of death remained undetermined despite post-mortem imaging.
