Finding into death of Maahi Tukapua

A 73-year-old man with significant comorbidities (cirrhosis, dilated cardiomyopathy, chronic kidney disease) died in prison from acute-on-chronic renal failure, hepatic encephalopathy, and multiorgan failure. He was initially reluctant to engage with hospital treatment due to concerns about handcuffing during appointments. Following initial conservative management, he was admitted to hospital twice in July 2021 for acute decompensation. He was then transferred to prison palliative care where he declined further investigations and opted for comfort measures only. The coroner found no issues with medical management during custody. Death was from natural causes with appropriate end-of-life care planning.
